Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner for the Southeast Buffalo NY area, with travel required to client and patient sites in and around Boston, NY. This unique opportunity offers external candidates a $5,000 sign-on bonus and provides the autonomy to perform comprehensive patient assessments without the heavy responsibility of managing ongoing orders, treatments, and follow-up care.

In this role, you will work as part of the Senior Community Care product team to support patients transitioning from a skilled rehab facility back home. Your primary focus will be conducting one-time comprehensive visits to ensure a smooth transition, prevent readmissions, capture STAR measures, educate patients, and accurately document all acute and chronic diagnoses in compliance with CMS regulations.

Key responsibilities include:

  • Performing comprehensive assessments and documenting findings concisely and accurately.
  • Ensuring all diagnoses are properly coded using ICD-10 and documented to support the visit.
  • Addressing and documenting all quality elements and conducting initial medication reviews.
  • Communicating pertinent findings and medical concerns to attending physicians and facility staff.
  • Completing all mandatory educational and virtual training requirements.
  • Managing scheduling via a web-based application and participating in quality initiatives.

To qualify for this position, candidates must meet the following requirements:

  • Active and unrestricted Nurse Practitioner license in the state of New York.
  • Master’s degree in Nursing (MSN) from an accredited program.
  • Board certification as a Nurse Practitioner through the AANP, ANCC, or other approved national board (preferred certifications include ANP, FNP, or GNP).
  • Proficient computer skills and experience with electronic medical records.
  • Ability to lift a 30-pound bag in and out of a vehicle, navigate stairs, and travel to various patient dwelling conditions.
  • Access to reliable transportation for travel within the designated service area.

Preferred qualifications include one or more years of clinical practice experience—particularly in long-term care settings—along with a strong understanding of geriatrics, chronic illness, and advanced illness discussions.

The hourly pay for this full-time role ranges from $43.22 to $77.21, determined by factors such as education, certifications, and local labor markets.
