Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner or Physician Assistant for our Senior Long-Term Care program serving Marion and Benton County, based out of Lake Oswego, OR. We are uniting industry-leading solutions to build an integrated care model that holistically addresses an individual’s physical, mental, and social needs—helping patients access and navigate care anytime and anywhere.
As part of our Senior Community Care (SCC) product, you will work within an interdisciplinary team to provide longitudinal, integrated clinical care to our highest-risk health plan members residing in facilities such as nursing homes, assisted living communities, or senior housing. Best of all, this role features no on-call shifts, no weekend requirements, and no holidays required.
Primary Responsibilities:
- Conduct comprehensive health assessments for high-risk patients.
- Manage members through acute medical situations or chronic condition exacerbations, including coordinating testing, diagnostics, and clinical interventions.
- Manage members during short-term sub-acute rehabilitation episodes.
- Ensure accurate and complete documentation, including ICD-10 conditions.
- Communicate and collaborate effectively with the interdisciplinary care team, patients, and caregivers.
- Conduct advanced illness and advanced care planning conversations, and provide ongoing counseling and education.
Required Qualifications for Nurse Practitioners:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted Nurse Practitioner license in the state of residence, or ability to obtain by start date.
- Board certified through the AANP or ANCC with certification in Family, Adult, Gerontology, Adult-Gerontology Acute Care, or Adult-Gerontology Primary Care.
Required Qualifications for Physician Assistants:
- Graduate of an accredited Physician Assistant degree program.
- Active and unrestricted PA license in the state of residence, or ability to obtain by start date.
- Board certified by the NCCPA.
- Current, active DEA licensure or ability to obtain post-hire per state regulations.
- Ability to gain a collaborative practice agreement if applicable, maintain a driver’s license with reliable transportation, and meet physical job requirements including managing equipment and navigating various dwelling conditions.
Preferred Qualifications:
- 1+ years of clinical practice experience, preferably within a long-term care setting.
- Experience addressing complex behavioral, social, and functional patient needs.
- Strong understanding of geriatrics, chronic illness, and end-of-life discussions.
- Proficient computer skills with experience in electronic medical records.
The annual salary for this full-time role ranges from $89,900 to $160,600, determined by factors such as education, certifications, and local labor markets.
