Nurse Practitioner or Physician Assistant, Senior Long-Term Care (Marion & Benton County)

Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Senior Long-Term Care team serving Marion and Benton County, based out of Albany, OR. As part of our Senior Community Care (SCC) program, you will provide longitudinal, integrated clinical care to our highest-risk health plan members residing in nursing homes, assisted living, or senior housing environments. Best of all, this role features no on-call shifts, no weekends, and no holidays required.

In this vital role, you will work collaboratively within an interdisciplinary care team to deliver comprehensive, patient-centered care. Your primary responsibilities will include:

• Conducting comprehensive clinical assessments and managing members through acute medical situations or chronic condition exacerbations, including ordering and interpreting diagnostic testing.
• Managing members during short-term sub-acute rehabilitation episodes.
• Ensuring accurate and complete medical documentation, including proper ICD-10 condition coding.
• Communicating and collaborating seamlessly with the interdisciplinary care team, patients, and caregivers.
• Conducting advanced illness and advanced care planning conversations, alongside providing supportive counseling and patient education.

To qualify for this position as a Nurse Practitioner, you must be a graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program, hold an active and unrestricted Nurse Practitioner license in your state of residence (or ability to obtain by start date), and maintain relevant board certification through the AANP or ANCC in specialties such as Family, Adult, Gerontology, or Adult-Gerontology Care. For Physician Assistants, you must be a graduate of an accredited PA degree program, hold an active and unrestricted PA license, maintain board certification by the NCCPA, and possess current active DEA licensure or the ability to obtain it post-hire. All candidates must have a driver’s license, access to reliable transportation, and the physical ability to carry out field-based responsibilities.

Preferred qualifications include one or more years of clinical practice experience, particularly within a long-term care setting, alongside a strong understanding of geriatrics, chronic illness, and end-of-life discussions.

The annual salary for this full-time role ranges from $89,900 to $160,600, with final compensation determined by factors such as education, certifications, and local labor markets.
