Optum is currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team in Albany, NY, serving Rensselaer County. As part of Optum Home & Community Care and the UnitedHealth Group family of businesses, we are building an integrated care model that holistically addresses patients’ physical, mental, and social needs. In this role within our Senior Community Care (SCC) program, you will provide longitudinal, integrated care to our highest-risk health plan members in facility settings such as nursing homes, assisted living, or senior housing.
This position offers a flexible schedule where providers can flex their time over the 7-day work week, with no on-call, no weekends, and no holidays required. Eligible individuals who have not previously participated in this program may qualify for a $40,000 student loan repayment or a $30,000 sign-on bonus, subject to specific active-status and retention guidelines. Compensation for this specialty generally ranges from $104,500 to $156,000, determined by factors such as local labor markets, education, and experience, alongside a comprehensive benefits package, incentive programs, and 401(k) contributions.
Primary responsibilities include:
- Conducting comprehensive assessments and managing members through acute medical situations or chronic condition exacerbations, including testing, diagnostics, and interventions.
- Managing members during short-term sub-acute rehabilitation episodes and ensuring accurate, complete documentation including ICD-10 conditions.
- Communicating and collaborating effectively with the interdisciplinary care team.
- Conducting advanced illness and advanced care planning conversations while providing patient and caregiver counseling and education.
Required qualifications include:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program, or equivalent PA training.
- Active and unrestricted license in the state of practice, or the ability to obtain by the start date.
- Active certification through a recognized national board (such as AANP or ANCC) in an applicable specialty, where required.
- Current, active DEA licensure and prescriptive authority or the ability to obtain post-hire.
- Ability to meet physical requirements, including moving a 30-pound bag in and out of a car, navigating stairs, and performing physical assessments.
- A valid driver’s license and access to reliable transportation.
Preferred qualifications include prior clinical experience in practice, particularly within a long-term care setting, experience working with patients with complex behavioral, social, or functional needs, a strong understanding of geriatrics, and proficient computer skills for electronic medical record documentation.
