Optum is seeking a dedicated Nurse Practitioner or Physician Assistant to join our team serving Dutchess and Ulster Counties, based in Poughkeepsie, NY. In this role within our Senior Community Care (SCC) program, you will provide longitudinal, integrated clinical care to high-risk health plan members in facility settings such as nursing homes, assisted living facilities, and senior housing. Primary Responsibilities:

  • Conduct comprehensive assessments and manage members through acute medical situations or chronic condition exacerbations, including testing, diagnostics, and interventions.
  • Manage members during short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Communicate and collaborate effectively with the interdisciplinary care team.
  • Conduct advanced illness and advanced care planning conversations.
  • Provide patients and caregivers with counseling and education.

Required Qualifications:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program, or equivalent PA training.
  • Active and unrestricted state license, or ability to obtain by the start date.
  • Active national Nurse Practitioner certification (such as AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute Care), or corresponding PA credentials.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire.
  • Ability to complete physical requirements, including moving a 30-pound bag in and out of a vehicle, navigating stairs and various dwelling conditions, and performing physical assessments.
  • Driver’s license and access to reliable transportation.

Preferred Qualifications:

  • Clinical experience in practice, particularly within a long-term care setting.
  • Experience meeting the medical needs of patients with complex behavioral, social, and functional needs.
  • Strong understanding of geriatrics, chronic illness, advanced illness, and end-of-life discussions.
  • Proficient computer skills, including electronic medical record documentation.

Compensation and Incentives:

  • Compensation generally ranges from $125,000 to $175,000, determined by factors such as local labor markets, education, and work experience.
  • Eligible for a $40,000 Student Loan Repayment or $30,000 Sign-on Bonus for individuals who have not previously participated in this program (subject to ongoing program terms and eligibility requirements).
