Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is looking for a Nurse Practitioner or Physician Accountant for a Long-Term Care role covering Multnomah and Clackamas County, operating out of Salem, OR. Join our Senior Community Care (SCC) product team to provide vital care to patients in home, nursing home, assisted living, or senior housing settings. Our mission is to build an integrated care model that holistically addresses physical, mental, and social needs, helping patients access and navigate care anywhere.
In this longitudinal, integrated care delivery program, Advanced Practice Clinicians (APCs) provide care to our highest-risk health plan members as part of an interdisciplinary team. Primary Responsibilities:
- Conduct comprehensive assessments and manage members through acute medical situations or chronic condition exacerbations, including testing, diagnostics, and interventions.
- Manage members during short-term sub-acute rehabilitation episodes.
- Ensure accurate and complete documentation, including ICD-10 conditions.
- Communicate and collaborate closely with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide patients and caregivers with counseling and education.
Required Qualifications:
- For Nurse Practitioners: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program; active and unrestricted NP license (or ability to obtain); board certification through AANP or ANCC in Family, Adult, Gerontology, Adult-Gerontology Acute Care, or Adult-Gerontology Primary Care Nurse Practitioner.
- For Physician Assistants: Graduate of an accredited PA degree program; active and unrestricted PA license (or ability to obtain); board certification by the NCCPA; current active DEA licensure/prescriptive authority (or ability to obtain post-hire).
- Ability to gain a collaborative practice agreement, if applicable.
- Ability to complete physical requirements including moving a 30-pound bag in and out of the car, navigating stairs, and performing physical assessments.
- Valid driver’s license and access to reliable transportation.
Preferred Qualifications:
- 1+ years of clinical experience in practice, with a long-term care setting preferred.
- Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of geriatrics, chronic illness, advanced illness, and end-of-life discussions.
- Proficient computer skills, including electronic medical record documentation.
Compensation for this full-time role ranges from $89,900 to $160,600 annually, determined by factors such as local labor markets, education, work experience, and certifications.
