PT – Physical Therapist

In this role, you will play a vital part in post-acute care by assessing, evaluating, and treating patients within their home environments. You will work closely with physicians and the broader healthcare team to improve patient mobility, manage pain, and enhance overall functional well-being.

Primary Responsibilities:

• Evaluates the patient’s functional status and physical therapy needs, and assists the physician in developing the therapy plan of care.
• Provides professional physical therapy services in accordance with state laws, the plan of care, and team coordination.
• Observes and reports patient responses to treatment and any condition changes to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum independence using acceptable standards of practice.
• Instructs and informs the care team, patient, and family members regarding the plan of care, limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment, completes timely progress notes, and submits documentation per company policy.
• Completes all patient evaluations and establishes the PT plan of care within state-specific guidelines, reporting outcomes and projected frequencies of care.

Required Qualifications:

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable transportation.
• Ability to function professionally in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Compensation is based on factors including local labor markets, education, experience, and certifications. The hourly pay for this role ranges from $34.23 to $61.15 per hour based on full-time employment.
