Optum is seeking a dedicated Licensed Vocational Nurse (LVN) to join our team in Allen, TX. In this role, you will be responsible for providing quality patient care and performing technical skilled care directly in the patient’s home. Your daily efforts will directly impact the well-being of our patients while working as part of a collaborative health care team.
Primary responsibilities include:
- Providing high-quality clinical services within your designated scope of practice, adhering to state nursing laws, the established plan of care, and infection control procedures.
- Documenting all patient visits thoroughly and completely according to regulatory and payer requirements using the agency’s electronic medical record system and electronic visit verification program.
- Participating actively in the agency’s care coordination process, including reporting changes in patient condition, attending case conferences, and communicating effectively with patients, families, physicians, and agency staff.
Required qualifications for this position include:
- Current and unrestricted LPN or LVN licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and dependable transportation or access to reliable public transportation.
- Ability to work flexible hours to meet client needs and function effectively in diverse home environments.
Preferred qualifications include prior home care experience, the ability to multitask and work independently, and strong communication, writing, and organizational skills.
The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with final compensation determined by factors such as local labor markets, education, experience, and certifications. Eligible employees may also participate in a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contributions, subject to eligibility requirements.
