Optum is seeking a Nurse Practitioner to join our Home-based Medical Care team for traveling in-home visits throughout the entire state of Alabama, based out of Homewood, AL. This role offers an opportunity to bring compassionate, high-end medical care directly to complex, chronic patients and older adults in their own homes.
As a statewide traveler, you will work as a provider member within an interdisciplinary team to deliver direct care. Your responsibilities will include:
- Performing initial comprehensive assessments on newly enrolled patients and providing ongoing care management.
- Developing, reviewing, and updating patient management care plans upon enrollment and following hospitalizations.
- Conducting basic nursing procedures such as IV placement, blood draws, injections, foley catheters, and wound care.
- Prescribing appropriate diagnostics and interventions to prevent unnecessary acute admissions.
- Educating patients and their legal representatives on disease management and wellness promotion.
- Implementing quality initiatives and HEDIS measure campaigns to maintain high standards of care delivery.
To qualify for this position, candidates must have completed an accredited Nurse Practitioner program, hold a current state RN and NP license, and possess certification through the AANP, ANCC, or NCCPA. Access to reliable transportation that complies with company safety policies is also required.
The salary range for this full-time role is $89,900 to $160,600 annually, depending on factors such as education, experience, and local labor markets. Additionally, eligible individuals who have not previously participated in this program may qualify for a $40,000 student loan repayment plan or a $25,000 sign-on bonus, subject to specific program terms and active employment duration requirements.
