In this role as a Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by a physician to acutely ill or chronically ill patients in their home environments. Working under the direct supervision of a Registered Nurse or designated supervisor, you will ensure the highest quality of patient care within your assigned geographic service area.

Primary Responsibilities:

  • Provide high-quality clinical services within your scope of practice, adhering to state nursing laws, the established plan of care, and infection control procedures.
  • Document patient visits thoroughly and completely per regulatory and payer requirements using the electronic medical record and electronic visit verification program.
  • Participate actively in the care coordination process by reporting patient needs and condition changes promptly, attending patient case conferences, and communicating effectively with patients, families, physicians, and agency staff.

Required Qualifications:

  • Current and unrestricted LPN licensure in the state of Louisiana.
  • Current CPR certification.
  • Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Ability to function professionally in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
  • 1+ years of experience working as an LPN.

Preferred Qualifications:

  • Home care experience.
  • Ability to work flexible hours to meet identified client needs and manage multiple tasks simultaneously.
  • Ability to work independently with strong communication, writing, and organizational skills.

Compensation and Benefits:

  • The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with pay determined by factors including local labor markets, education, work experience, and certifications.
