As a Licensed Practical Nurse in Home Health, you will be responsible for providing quality patient care and performing technical skilled care directly in the patient’s home. You will play a vital role in delivering patient-centered care and supporting positive health outcomes within the community.
Primary Responsibilities:
- Provide high quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other members of the health care team while following all applicable infection control procedures.
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency’s electronic visit verification program, as applicable.
- Actively participate in the agency’s care coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management staff, case coordinators, and community case managers.
Required Qualifications:
- Current and unrestricted LPN licensure in the state of practice (specifically, current LPN licensure in Louisiana without restrictions, along with a minimum of one year of experience working as an LPN for candidates in this state).
- Current CPR certification.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable transportation.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Preferred Qualifications:
- Home care experience.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
Compensation and Benefits:
- The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with pay determined by factors such as local labor markets, education, work experience, and certifications.
