As the Senior Living Physical Therapist, you will provide age-appropriate services for adult to geriatric patients, involving direct patient care. Your core responsibilities will include:

• Evaluating the patient to determine rehab potential, identifying needs, establishing an individualized treatment plan of care, and setting short-term and long-term goals to be used in the team plan of care.
• Providing comprehensive assessment, direct treatment to individuals and groups, supervision of treatment delivered by the PTA or tech, development of a home exercise program, and patient/family instruction.
• Participating in team and family conferences, evaluating and selecting adaptive equipment, participating in performance improvement programs, and documenting services according to department and facility standards.
• Utilizing equipment in a proper and safe manner, maintaining equipment inspection, maintenance, and cleaning logs, and reporting broken or faulty equipment.
• Following the medical regimen developed for the patient while remaining aware of and adhering to specific precautions such as THR, TKR, Cardiac, and Wound care.
• Utilizing safe treatment techniques including transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, ultrasound, and electrical stimulation.
• Completing discharge summaries on each patient on their final visit to outpatient physical therapy.

Required qualifications for this role include current Physical Therapy licensure in the state of practice, current CPR certification, a proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client, and a current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Pay is based on several factors including local labor markets, education, work experience, and certifications. The hourly pay for this full-time role ranges from $34.23 to $61.15 per hour.
