Metairie LA PCS, part of LHC Group and the Optum family of businesses, is seeking a dedicated Licensed Practical Nurse (LPN) for pediatric home care opportunities in the New Orleans area with a flexible schedule. As a Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care under the supervision of a Registered Nurse and/or designated supervisor. You will care for acutely or chronically ill patients in their home environments, ensuring the highest quality of care throughout your assigned geographic service area.
Primary responsibilities for this role include:
- Providing high-quality clinical services within your licensed scope of practice and in accordance with the established plan of care and state laws.
- Documenting patient visits thoroughly and completely per regulatory and payer requirements using the electronic medical record and electronic visit verification systems.
- Participating actively in care coordination by reporting patient needs and condition changes promptly, attending case conferences, and communicating effectively with patients, families, physicians, and care team members.
Required qualifications include:
- Current and unrestricted LPN licensure in the state of Louisiana (with 1+ years of experience working as an LPN).
- Current CPR certification.
- Current driver’s license, vehicle insurance, access to a dependable vehicle or reliable public transportation, and the ability to function in diverse home environments.
Preferred qualifications include prior home care experience, the ability to work flexible hours to meet client needs, strong multitasking and independent working skills, and good communication abilities. The hourly pay for this role ranges from $20.00 to $35.72 per hour, based on factors such as local labor markets, education, experience, and certifications. Eligible team members may also have access to a comprehensive benefits package, incentive programs, and retirement contributions, subject to standard eligibility requirements.
