Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Senior Community Care (SCC) product in Rochester. We are uniting industry-leading solutions to build an integrated care model that holistically addresses an individual’s physical, mental, and social needs, helping patients access and navigate care anytime and anywhere. To support our team members, we are pleased to offer a $30,000 sign-on bonus or $40,000 in student loan payments.

As part of our SCC program, you will work within an interdisciplinary team to provide longitudinal, integrated clinical care to our highest-risk health plan members in facility settings, such as nursing homes, assisted living facilities, or senior housing. This role offers an excellent opportunity to achieve work-life balance through flexible scheduling that allows providers to flex their time over the seven-day work week, with no on-call duties, no weekends, and no holidays required.

Primary responsibilities for this role include:

  • Conducting comprehensive assessments and managing members through acute medical situations or chronic condition exacerbations, including testing, diagnostics, and interventions.
  • Managing members during short-term sub-acute rehabilitation episodes.
  • Ensuring accurate and complete documentation, including ICD-10 conditions.
  • Communicating and collaborating effectively with the interdisciplinary care team.
  • Conducting advanced illness and advanced care planning conversations.
  • Providing patients and caregivers with essential counseling and education.

Required qualifications include graduation from an accredited Master of Science Nursing or Doctor of Nursing Practice program, an active and unrestricted state license (or the ability to obtain by the start date), and active national board certification as a Nurse Practitioner (such as Family, Adult, Gerontology, or Adult-Gerontology Acute Care). Candidates must also hold a current active DEA licensure or prescriptive authority (or the ability to obtain post-hire), the ability to gain a collaborative practice agreement if applicable, a driver’s license with access to reliable transportation, and the physical capability to meet job demands, such as moving a 30-pound bag, navigating stairs, and performing patient assessments. Preferred qualifications include one or more years of clinical practice experience, particularly in long-term care settings, and familiarity with geriatrics, chronic illness, and electronic medical records.

Compensation for this role generally ranges from $104,500 to $156,000, which includes base pay and is determined by factors such as local labor markets, education, and work experience. Additional rewards include comprehensive benefits, incentive and recognition programs, equity stock purchase, and 401(k) contributions subject to eligibility requirements.
