Optum Home & Community Care is seeking a dedicated Nurse Practitioner (NP) to join our Optum Care at Home team, serving patients across McDowell, Buncombe, and Burke counties in North Carolina. In this role, you will be part of an interdisciplinary care team dedicated to delivering direct, in-home medical care and preventive health services. This position offers a schedule with no nights, weekends, or holidays, and requires daily travel to patient homes within your assigned geographic area.

As an NP with Optum Care at Home, your primary responsibilities will include:

  • Performing initial comprehensive assessments of newly enrolled patients and providing ongoing medical care.
  • Developing and updating patient management care plans upon enrollment and following any changes in condition or hospitalization.
  • Meeting with patients and legal representatives to review care plans in collaboration with primary care physicians.
  • Executing basic nursing and clinical procedures such as IV placement, blood draws, injections, wound care, and catheter management.
  • Prescribing appropriate diagnostics and interventions to prevent unnecessary acute hospital admissions.
  • Educating patients and their families on disease processes, wellness promotion, and effective self-management.
  • Participating in quality initiatives, clinical meetings, and continuing education to maintain high standards of care.

We are looking for professionals who meet the following required qualifications:

  • Current state RN/NP license or the ability to obtain it by your start date.
  • Certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Current active DEA licensure and prescriptive authority or the ability to obtain it post-hire per state regulations.
  • A valid driver’s license, access to reliable transportation, and compliance with company motor vehicle safety policies.

Preferred qualifications for this role include two or more years of clinical NP experience in internal medicine, geriatrics, primary care, emergency medicine, urgent care, or home health care, along with strong organizational and computer literacy skills.

Qualified individuals may be eligible for a $40,000 student loan repayment program or a $20,000 sign-on bonus for those who have not previously participated in the program, subject to specific retention guidelines and eligibility terms. Compensation for this specialty generally ranges from $109,500 to $164,000, determined by factors such as local labor markets, education, and experience, alongside a comprehensive benefits package, 401(k) contributions, and incentive programs.
