As a Physical Therapist in Home Health, you will complete comprehensive assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on your evaluations, you will collaborate to determine an effective treatment plan, perform interventions aimed at enhancing patient well-being, and continuously evaluate patient progress.

Primary Responsibilities:

• Evaluates the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
• Provides services within the scope of practice defined by state laws governing physical therapy, in accordance with the care plan and in coordination with the health care team.
• Observes and reports the patient’s response to treatment and any condition changes to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
• Instructs and informs the health care team, the patient, and family caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment, completing and submitting progress notes for each visit per company policy.
• Completes all patient evaluations and develops the physical therapy plan of care within state-specific guidelines, reporting outcomes, goals, and projected frequency of care.

Required Qualifications:

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
• Ability to function professionally in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Pay is based on several factors including local labor markets, education, work experience, and certifications. The hourly pay for this full-time role ranges from $34.23 to $61.15 per hour.
