In this vital role, you will provide direct nursing care as prescribed by the physician to acutely or chronically ill patients within their home environments. Working under the direct supervision of a Registered Nurse or designated supervisor, you will ensure the highest quality of patient care, coordinate related health services, and support patients and their families throughout the geographic service area.
Primary responsibilities include:
- Delivering high-quality clinical services within your licensed scope of practice, adhering to the established plan of care and agency infection control procedures.
- Documenting patient visits thoroughly and accurately per regulatory and payer requirements using the electronic medical record and electronic visit verification program.
- Participating actively in care coordination by reporting patient needs and changes in condition promptly, attending case conferences, and communicating effectively with patients, families, physicians, and agency management.
Required qualifications for this position:
- Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle.
- Ability to function professionally and compassionately in any home situation, regardless of client background, age, race, or financial condition.
Preferred qualifications that will help you succeed include prior home care experience, the ability to work flexible hours and manage multiple tasks simultaneously, strong independent working capabilities, and excellent communication, writing, and organizational skills.
The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with exact compensation determined by factors such as local labor markets, education, work experience, and certifications.
