As a Physical Therapist in Home Health, you will play a vital role in completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on your evaluations, you will work to determine treatment plans, perform targeted interventions, and evaluate patient progress to enhance overall well-being.
Primary responsibilities for this role include:
- Evaluating the patient’s functional status and physical therapy needs, and assisting the physician in developing the therapy plan of care.
- Providing services within your professional scope of practice and in coordination with other members of the healthcare team.
- Observing and reporting the patient’s response to treatment and any condition changes to the supervising nurse or physician.
- Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instructing and informing patients, families, caregivers, and team members regarding the plan of care, functional limitations, and progress toward goals.
- Visiting patients according to the plan of treatment, completing progress notes for each visit, and submitting documentation according to company policy.
- Completing all patient evaluations and developing the physical therapy plan of care within state-specific guidelines.
Required qualifications to be considered for this position include:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
- Ability to function professionally in any home situation regardless of client background, age, race, or financial condition.
This position features a $5,000 sign-on bonus. The annual total cash target pay ranges from $77,400 to $171,000, incorporating per visit point pay and hourly rates depending on the specific tasks performed. Additional offerings include a comprehensive benefits package, recognition programs, equity stock purchase, and 401(k) contributions subject to eligibility requirements. Candidates are required to pass a drug test before beginning employment in our drug-free workplace.
