In this role, you will provide direct nursing care as prescribed by a physician to acutely or chronically ill patients in their home environments. Working under the direct supervision of a Registered Nurse or designated supervisor, you will ensure the highest quality of patient care throughout your assigned geographic service area.
Primary responsibilities include:
- Providing high-quality clinical services within your licensed scope of practice and in accordance with the established plan of care and state nursing laws.
- Thoroughly and completely documenting patient visits per regulatory and payer requirements using the electronic medical record and electronic visit verification program.
- Participating actively in the agency’s care coordination process, including timely reporting of patient needs, communicating with physicians and families, and attending patient case conferences.
Required qualifications for this position include:
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable public transportation.
- The ability to function professionally in any home environment regardless of client background, age, or financial condition.
Preferred qualifications include prior home care experience, the ability to work flexible hours to meet client needs, strong multitasking and independent work capabilities, and good communication and organizational skills.
The hourly pay for this role ranges from $20.00 to $35.72 per hour, based on full-time employment and factors such as local labor markets, education, experience, and certifications.
