In this role, you will provide individualized, one-on-one care in home settings, helping patients receive the support they need to live independently and safely in their own residences. This position features a convenient daytime schedule from Monday through Friday, offering professional autonomy and schedule flexibility across a territory that includes Winona, Wabasha, Buffalo, and Trempealeau counties.
Essential job functions and responsibilities include:
- Perform initial and ongoing comprehensive assessments of patient needs and functional abilities
- Develop and implement individualized occupational therapy treatment plans tailored to the home environment
- Help patients improve their ability to perform daily activities and achieve greater independence
- Educate patients, family members, and caregivers on safe practices, therapeutic techniques, and the use of adaptive equipment
- Collaborate closely with other healthcare team members to ensure coordinated, high-quality patient care
- Maintain accurate and timely clinical documentation in accordance with agency standards
