- Collaborating closely with registered nurses, physicians, and therapists to develop and update patient care plans.
- Educating patients and their families on post-discharge care, wellness strategies, and ongoing health management.
- Maintaining accurate, up-to-date medical documentation in compliance with facility standards and healthcare regulations.
The annual salary for this full-time position is established at $76,704 per year.
