Trinity Health is seeking a dedicated and patient-centered Family Medicine Physician to join the dynamic team at Capital District Family Medicine, located at 1440 Western Avenue in Albany, NY, serving the Upper Union and broader Schenectady County area. This opportunity is available on either a full-time or part-time basis, allowing for flexible scheduling to suit your professional lifestyle.
In this role, you will be part of an established outpatient practice that benefits from strong patient demand and comprehensive hospital-affiliated practice support. You will work alongside a collaborative team of experienced healthcare providers committed to delivering high-quality, comprehensive care to individuals and families throughout the community.
Key responsibilities for this position typically include:
- Providing comprehensive, continuous primary medical care to patients of all ages
- Conducting routine check-ups, health assessments, and preventative screenings
- Diagnosing and treating acute and chronic illnesses
- Developing and managing personalized patient treatment plans
- Ordering and interpreting diagnostic tests such as lab work and imaging
- Collaborating with specialists and interdisciplinary healthcare teams to ensure coordinated patient care
- Maintaining accurate and timely electronic health record documentation
The ideal candidate will possess exceptional clinical skills, a strong commitment to patient-centered care, and the ability to foster trusting relationships with patients and colleagues alike. Candidates must hold the appropriate medical degrees, licensures, and board certifications required to practice family medicine in the state of New York.
