- Assessing, planning, implementing, and evaluating individualized patient care plans
- Administering medications and treatments in accordance with physician orders and facility protocols
- Monitoring vital signs and clinical changes, taking prompt action as needed
- Educating patients and their families on post-discharge care, disease management, and recovery strategies
- Collaborating with physical therapists, occupational therapists, physicians, and other healthcare team members to ensure coordinated, holistic care
- Maintaining accurate and timely electronic health records
