CHRISTUS Health is seeking a dedicated and experienced Acute Care Nurse Practitioner to join our dynamic cardiology team. In this role, you will play a vital part in delivering high-quality, patient-centered cardiovascular care within a collaborative healthcare environment, serving patients across the Santa Fe, New Mexico area.
As an Acute Care Nurse Practitioner, your day-to-day responsibilities will include a balanced mix of inpatient and outpatient care, working closely with our cardiology physicians to ensure comprehensive treatment for every patient. Your core duties will involve:
- Performing thorough patient assessments, comprehensive medical histories, and physical examinations in both inpatient and outpatient settings.
- Managing a wide range of acute and chronic cardiovascular conditions in alignment with established clinical guidelines.
- Ordering, interpreting, and appropriately following up on diagnostic studies and laboratory tests.
- Assisting with daily hospital rounds, patient admissions, and discharge planning.
- Developing, executing, and continuously evaluating individualized treatment plans in close collaboration with supervising cardiologists.
- Providing vital education to patients and their families regarding heart health, ongoing disease management, and necessary lifestyle modifications.
- Documenting all patient care and clinical encounters accurately and in a timely manner within the electronic medical record (EMR) system.
To be considered for this full-time opportunity, candidates must possess the appropriate state licensure for an Acute Care Nurse Practitioner, along with the relevant national certification. A strong background in cardiology or inpatient acute care is required, supported by sound clinical judgment, exceptional critical thinking skills, and the ability to thrive in a fast-paced, multidisciplinary healthcare setting.
