Aveanna Healthcare is seeking a dedicated and compassionate Home Health Occupational Therapist (OT) to join our growing team on a full-time basis. In this role, you will utilize your clinical expertise to assess, teach, and deliver therapy interventions and treatments directed by the physician as established in the comprehensive home health plan of care.
You will work closely with patients in their home environments within your assigned coverage area, which includes DeKalb and South Gwinnett counties. Your schedule will consist of full-time visits scheduled Monday through Friday, allowing you to provide consistent, one-on-one care that truly makes a difference in the lives of those we serve.
Key responsibilities for this position include:
- Delivering skilled occupational therapy interventions in accordance with physician orders and established home health care plans.
- Conducting thorough patient assessments to evaluate functional abilities, home safety, and progress toward therapy goals.
- Educating patients, families, and caregivers on adaptive techniques, energy conservation, and the proper use of therapeutic equipment.
- Collaborating closely with an interdisciplinary healthcare team, sharing and receiving pertinent updates to optimize patient well-being and recovery outcomes.
- Maintaining accurate, timely documentation of patient visits, clinical observations, and progress notes in compliance with regulatory and company standards.
