In this role, you will be completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. You will help determine a treatment plan, perform interventions to enhance patient well-being, and evaluate ongoing progress.
Primary responsibilities include:
- Evaluating the patient’s functional status and physical therapy needs to assist the physician in developing a plan of care.
- Providing services within your state scope of practice, in coordination with other healthcare team members.
- Observing and reporting patient responses to treatment and condition changes to the supervising nurse and/or physician.
- Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instructing patients, families, caregivers, and healthcare team members regarding the plan of care, limitations, and progress toward goals.
- Completing timely progress notes for each visit and finalizing evaluations and care plans within state-specific guidelines.
Required qualifications:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and dependable transportation (or reliable public transportation).
- Ability to function professionally in any home situation regardless of the client’s age, race, background, or financial condition.
The annual total cash target pay range for this position is $77,400 to $171,000, consisting of a per visit point rate of $44.65 to $98.65 and an hourly rate of $37.21 to $82.21 for non-visit activities. Total compensation varies based on factors such as local labor markets, education, experience, productivity, and performance.
