At AHN Healthcare@Home, we are looking to add an extraordinary Hospice LPN to our care team in Cranberry Township, PA, supporting patients in North Side, PA and surrounding areas. Grounded by our belief that outstanding care is best delivered in a team-based environment, our Hospice LPNs collaborate to provide the management and delivery of patient care plans. In this role, your daily responsibilities will center on delivering compassionate nursing care to hospice patients in their homes. You will work closely with the broader care team to execute patient care plans, consistently meet critical deadlines for charting, and utilize your strong time-management skills to self-manage your daily schedule. Mobile technology is a key part of the workflow, and you will use company-provided tablets and smartphones along with EMR software to document care efficiently.
We are looking for compassionate nurses who meet the following qualifications:
- Hold a valid LPN license in the state where you work
- Be a graduate from an approved school of practical nursing
- Bring two years of LPN experience, with hospice experience preferred
- Possess a current driver’s license and the ability to spend approximately 20% of your day driving to and from patient locations
- Demonstrate a commitment to consistently meet critical deadlines for charting
- Possess the skills needed to effectively self-manage your time and schedule
- Demonstrate experience using tablets, mobile phones, and EMR software
