We are currently seeking a dedicated Licensed Practical Nurse (LPN) for pediatric home care night shifts in the Stonewall area to provide compassionate, high-quality care to patients in their homes.
As a Home and Community Based Services Licensed Practical Nurse, you will deliver direct nursing care as prescribed by a physician to acutely or chronically ill patients. Working under the direct supervision of a Registered Nurse or designated supervisor, you will play a vital role in ensuring patients receive exceptional support in their home environment.
Primary responsibilities for this role include:
- Providing clinical services within your licensed scope of practice and in accordance with the established plan of care.
- Documenting patient visits thoroughly and completely within the electronic medical record, utilizing electronic visit verification programs as applicable.
- Participating actively in the agency’s care coordination process, including reporting changes in patient condition and communicating effectively with families, physicians, and health care team members.
Required qualifications for this position include:
- Current and unrestricted LPN licensure in the state of Louisiana.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to dependable transportation.
- One or more years of experience working as an LPN.
- The ability to function professionally in diverse home environments regardless of client background or condition.
Preferred qualifications include previous home care experience, the ability to work flexible hours and manage multiple tasks simultaneously, strong independent working skills, and excellent communication abilities.
The hourly pay for this role ranges from $20.00 to $35.72 per hour, determined by factors such as local labor markets, education, experience, and certifications.
