Minden LA PCS, part of LHC Group and the Optum family of businesses, is seeking a dedicated Licensed Practical Nurse for pediatric home care in the Rayville area. As the Home and Community Based Services Licensed Practical Nurse, you will deliver direct nursing care as prescribed by a physician to acutely or chronically ill patients within their homes. Working under the direct supervision of a Registered Nurse or designated supervisor, you will record clinical and progress notes and coordinate patient care activities to ensure the highest quality of service in your assigned geographic area.
Primary responsibilities include:
- Providing high-quality clinical services within your state-defined scope of practice, adhering to the plan of care, coordination guidelines, and infection control procedures
- Documenting patient visits thoroughly within the electronic medical record and utilizing the agency’s electronic visit verification program as applicable
- Participating actively in care coordination by reporting patient needs and condition changes promptly, attending case conferences, and communicating effectively with patients, families, physicians, and care team members
Required qualifications include:
- Current and unrestricted LPN licensure in the state of Louisiana with at least 1 year of experience working as an LPN
- Current CPR certification
- Current driver’s license, vehicle insurance, and reliable access to a dependable vehicle or public transportation
- Ability to function professionally in diverse home environments regardless of client age, race, creed, color, sex, disability, or financial condition
Preferred qualifications include prior home care experience, the ability to work flexible hours to meet client needs, strong multi-tasking and independent working capabilities, and solid communication, writing, and organizational skills.
Compensation for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with pay determined by factors such as local labor markets, education, experience, and certifications.
