As a Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care prescribed by a physician to acutely or chronically ill patients in their homes. Operating under the direct supervision of a Registered Nurse or designated supervisor, you will ensure high-quality patient care within your assigned geographic area.
Primary Responsibilities include:
- Providing high-quality clinical services within your licensed scope of practice and in accordance with the established plan of care.
- Documenting patient visits thoroughly and completely per regulatory and payer requirements using the electronic medical record and electronic visit verification program.
- Participating actively in care coordination, including timely reporting of patient needs, condition changes, and attending patient case conferences.
- Communicating effectively with patients, families, physicians, other clinicians, and agency management staff.
Required Qualifications include:
- Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
- Ability to function in diverse home environments regardless of client age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications include:
- Previous home care experience.
- Ability to work flexible hours to meet client needs.
- Strong multitasking, independent working, and organizational skills.
- Good communication and writing abilities.
The hourly pay for this role ranges from $20.00 to $35.72 per hour, based on factors such as local labor markets, education, work experience, and certifications. Candidates are required to pass a drug test prior to beginning employment.
