As a Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by a physician to acutely or chronically ill patients in their home environments. Working under the direct supervision of a Registered Nurse or designated supervisor, you will record clinical progress notes, coordinate patient care activities, and help deliver the highest quality of care within your assigned geographic service area.
Primary responsibilities include:
- Providing high-quality clinical services within your state-defined scope of practice and in accordance with the established plan of care and infection control procedures.
- Documenting patient visits thoroughly and completely in compliance with regulatory, payer, and electronic medical record requirements.
- Actively participating in care coordination by reporting patient needs and changes in condition, attending case conferences, and communicating effectively with patients, families, physicians, and agency management.
Required qualifications for this position include:
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and reliable access to a dependable vehicle or public transportation.
- The ability to function professionally in any home situation regardless of client age, race, creed, color, sex, disability, or financial condition.
Preferred qualifications that enhance your candidacy include prior home care experience, the ability to work flexible hours and manage multiple tasks simultaneously, strong independent working capabilities, and excellent communication, writing, and organizational skills.
Compensation for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with pay determined by factors such as local labor markets, education, work experience, and certifications. Eligible employees may also have access to a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contributions, subject to eligibility requirements.
