As a Home and Community Based Services Licensed Practical Nurse focusing on weekend pediatric care in Metairie, you will provide direct nursing care as prescribed by a physician to acutely or chronically ill patients in their homes. Working under the direct supervision of a Registered Nurse or designated supervisor, you will record clinical and progress notes and coordinate other patient care activities to deliver the highest quality of care in your assigned geographic service area.
Primary Responsibilities:
- Provide high-quality clinical services within your licensed scope of practice, following the plan of care and coordinating with the broader healthcare team while adhering to infection control procedures.
- Document patient visits thoroughly per regulatory and payer requirements in the electronic medical record, utilizing the agency’s electronic visit verification program where applicable.
- Actively participate in the care coordination process by reporting patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and agency management.
Required Qualifications:
- Current and unrestricted Licensed Practical Nurse licensure in the state of Louisiana.
- 1+ years of experience working as an LPN.
- Current CPR certification.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function professionally in any home situation regardless of client background, age, race, or financial condition.
Preferred Qualifications:
- Previous home care experience.
- Ability to work weekend hours as required to meet identified client needs.
- Strong multitasking, independent working, and organizational skills.
The hourly pay for this role ranges from $20.00 to $35.72 per hour, with actual compensation based on factors including local labor markets, education, work experience, and certifications.
