In this hybrid role, you will be embedded within our healthcare client, Summit Medical Group, operating as a core member of the collaborative care team.
As a Mental Health Therapist, you will provide direct clinical services to patients within the Collaborative Care Model (CoCM) in a primary care clinic. This position offers a convenient Monday through Friday schedule with no evenings, weekends, or holidays required. You will enjoy a flexible hybrid work model, working remotely one to two days per week and onsite at the medical clinic for the remaining days.
Primary Responsibilities:
- Provide direct clinical services to patients using both virtual and in-person modalities, coordinating care with medical providers and other mental health professionals.
- Screen and assess patients for common mental health and substance abuse disorders.
- Systematically track treatment responses, monitor clinical symptoms, and observe potential treatment side effects or complications.
- Support psychotropic medication management prescribed by medical providers by monitoring treatment adherence, effectiveness, and side effects.
- Deliver brief behavioral interventions utilizing evidence-based techniques such as cognitive behavioral therapy, motivational interviewing, problem-solving treatment, and behavioral activation.
- Participate in regularly scheduled caseload consultations with psychiatric consultants and communicate treatment recommendations to medical providers.
- Develop and complete relapse prevention self-management plans for patients achieving their treatment goals.
Required Qualifications:
- Meet applicable state of Tennessee licensure requirements as an LCSW, LMSW, LAPSW, LMHC, LPC-MHSP (or temporary), or LMFT (or temporary).
- 3+ years of experience treating patients with behavioral health conditions.
- 1+ years of experience working in a primary care or other medical behavioral integrated setting.
- Demonstrated experience with assessment and evidence-based psychosocial treatments and brief behavioral interventions.
Preferred Qualifications:
- Experience working within the Collaborative Care Model (CoCM).
- Strong organizational skills, attention to detail, and exceptional follow-up capabilities.
- Proven ability to build effective professional relationships with patients and interdisciplinary care teams.
- Familiarity with providing care in-person as well as via telephone and video, with a solid grasp of healthcare technology solutions.
