Optum is seeking a dedicated Nurse Practitioner for our HouseCalls team in a Regional Traveler role based out of Leawood, KS, with multi-state travel required across Minnesota, Missouri, North Dakota, Nebraska, South Dakota, and Wisconsin. As part of our clinician-led care organization, you will help create a seamless health journey for patients by bringing high-end medical service, compassionate care, and preventive solutions directly to vulnerable patient populations.

In this role, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks while coordinating appropriate follow-up care. Enjoy a flexible schedule where you can flex your time over the 7-day work week, with no on-call duties, no weekends, and no holidays required.

Primary Responsibilities:

  • Conduct in-home assessments including past medical history review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, such as point of care testing where appropriate.
  • Identify and formulate a list of current and past medical conditions to guide care management and treatment.
  • Communicate assessment findings to inform members’ primary care physicians of potential gaps in care.
  • Identify urgent and emergent situations, intervening appropriately and educating members on topics like disease processes and medication compliance.

Required Qualifications:

  • Active Nurse Practitioner license or ability to obtain by start date, unencumbered and free of disciplinary actions.
  • Active national NP certification or ability to obtain by start date (ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care).
  • Ability to complete physical requirements, including lifting a 30-pound bag, navigating stairs, and performing physical assessments.
  • Access to reliable transportation for travel up to 85% of the time across assigned states to complete home visit assessments.

Preferred Qualifications:

  • 1+ years of clinical experience in a clinical setting.
  • Experience in Gerontology, Cardiology, Internal Medicine, Endocrinology, or Home Health care.

Compensation and Incentives:

  • The annual salary range for this full-time role is $89,900 to $160,600, depending on factors such as experience, education, and local labor markets.
  • Eligible individuals may qualify for a $40,000 Student Loan Repayment or a $30,000 Sign-on Bonus (subject to program terms, eligibility, and service commitments).
  • Comprehensive benefits package including incentive programs, equity stock purchase, 401(k) contributions, and covered travel expenses.
