About this job

Optum is seeking a Nurse Practitioner or Physician Assistant to join our Home-based Medical Care team covering Chautauqua, Cattaraugus, and Erie Counties, with work based in Jamestown, NY. As part of our clinician-led care organization, you will help bring home-based medical care to complex, chronic patients, providing life-changing work that helps older adults remain safely in their homes.

In this role, you will function as a provider member within our interdisciplinary team to deliver direct home-based medical care across an assigned geography. You will collaborate with primary care providers, interdisciplinary teammates, consultants, and medical directors to develop and carry out comprehensive patient care plans.

Key responsibilities include:

  • Practicing in accordance with state laws and regulations governing advanced practice nursing or physician assistant practice.
  • Performing initial comprehensive assessments on newly enrolled members and conducting bi-annual or follow-up assessments as needed.
  • Developing, reviewing, and updating member management care plans in coordination with patients, legal representatives, and primary care providers.
  • Prescribing appropriate diagnostics and interventions to support patient health and avoid unnecessary acute admissions.
  • Educating members, legal representatives, and facility staff on disease processes, wellness promotion, and effective care management.
  • Participating in routine case conferences, staff meetings, and quality improvement initiatives as directed.
  • Maintaining up-to-date knowledge on relevant medical and nursing research through continuing education and professional development.

Required qualifications:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program.
  • Current state RN license (if applying as a Nurse Practitioner).
  • Current NP or PA license.
  • Certification through the AANP, ANCC, or NCCPA.
  • Familiarity with long-term care, community resources, and cost-effective care delivery systems.
  • A valid driver’s license and access to reliable transportation.

Preferred qualifications:

  • Two or more years of clinical experience.
  • Familiarity with CPT, ICD-9, and HCPC codes.
  • Knowledge of clinical standards of care, NCQA requirements, CMS guidelines, and Medicare/Medicaid contracts.

Compensation for this role generally ranges from $104,500 to $156,000, determined by factors such as local labor markets, education, and experience. Additionally, eligible individuals may qualify for a $40,000 student loan repayment program or a $30,000 sign-on bonus if they have not previously participated in the program, subject to specific terms and retention requirements.
