Optum is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Optum Care At Home team in Chautauqua and Cattaraugus Counties, NY. As part of our clinician-led organization, you will help bring high-quality, home-based medical care directly to complex, chronic, and vulnerable patient populations. This rewarding role allows you to address the physical, mental, and social needs of older adults, helping them achieve more days at home and creating a seamless health journey.

In this role, you will work as a provider member within an interdisciplinary team to deliver direct home-based medical care across your assigned geography. You will collaborate closely with primary care physicians, interdisciplinary teammates, consultants, and medical directors to develop, implement, and update comprehensive patient care plans.

Key responsibilities include:

  • Performing initial comprehensive assessments on newly enrolled members and conducting bi-annual or follow-up assessments as needed.
  • Developing, reviewing, and updating member management care plans and involving primary care providers in the process.
  • Prescribing appropriate diagnostics and interventions to support patient health and avoid unnecessary hospital transfers or acute admissions.
  • Educating members, legal representatives, and facility staff on disease processes, wellness promotion, and effective care management.
  • Participating in routine management meetings, case conferences, quality improvement initiatives, and continuing education programs.
  • Managing caseloads, providing on-call support during business hours, and contributing to clinical on-call programs as the plan expands.

Required qualifications for this position:

  • Successful completion of an accredited Nurse Practitioner or Physician Assistant program.
  • Current state Registered Nurse license (if applying as a Nurse Practitioner) alongside a current NP or PA license.
  • Certification through the AANP, ANCC, or NCCPA.
  • Current active DEA licensure and prescriptive authority, or the ability to obtain it post-hire per state regulations.
  • Familiarity with long-term care policies, community resources, and cost-effective care delivery systems.
  • A valid driver’s license, reliable transportation, and compliance with company motor vehicle safety policies.

Preferred qualifications include two or more years of clinical experience, familiarity with medical coding standards, and an awareness of CMS guidelines, NCQA requirements, and Medicare/Medicaid contracts.

Compensation for this role generally ranges from $109,500 to $164,000, determined by factors such as local labor markets, education, and experience. Additionally, eligible candidates may qualify for a $40,000 student loan repayment program or a $30,000 sign-on bonus for individuals who have not previously participated in the program. Comprehensive benefits, retirement contributions, and incentive programs are also available.
