Nurse Practitioner or Physician Assistant – Optum Care At Home – Monroe County, New York

Optum is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Home-based Medical Care team serving Wayne, Ontario, and Seneca Counties, NY, as part of our broader efforts across Monroe County and surrounding regions. As a clinician-led care organization, we are committed to creating a seamless health journey for vulnerable patient populations, helping older adults with complex, chronic conditions live more days at home through compassionate, high-end medical service.

In this role, you will function as a vital provider member within the Home-Based Medical Care interdisciplinary team, delivering direct medical care within an assigned geography. You will collaborate closely with primary care physicians, interdisciplinary teammates, consultants, and health plan professionals to develop and implement holistic goals of care that address the physical, mental, and social needs of our patients.

Key responsibilities include:

• Practicing in accordance with state laws and regulations governing advanced practice nursing and physician assistant services, while complying with long-term care policies and procedures.
• Establishing and maintaining effective relationships with members, legal representatives, facility staff, and collaborating providers.
• Performing initial comprehensive assessments on newly enrolled members and bi-annually thereafter, developing and updating member management care plans as conditions warrant or following hospitalizations.
• Completing follow-up and post-hospital assessments, prescribing appropriate diagnostics and interventions to prevent unnecessary transfers or acute admissions, and consulting with hospital physicians upon member transfer.
• Educating members, legal representatives, and facility staff on disease processes, wellness promotion, and effective self-management strategies.
• Participating in regular meetings including weekly case conferences, monthly staff meetings, and quality improvement sessions, while implementing HEDIS measure campaigns and quality initiatives.
• Maintaining availability for on-call duties for your caseload during business hours, with potential participation in clinical on-call programs as the plan expands.

Required qualifications:

• Completion of an accredited Nurse Practitioner or Physician Assistant program.
• Current state RN license (if applying as a Nurse Practitioner) and current NP or PA license.
• Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
• Knowledge of long-term care, community resources, and cost-effective patient care delivery systems.
• Access to reliable transportation and compliance with the Landmark Motor Vehicle Safety policy.

Preferred qualifications:

• Two or more years of clinical experience.
• Familiarity with CPT, ICD-9, and HCPC codes.
• Understanding of clinical standards of care, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts and benefit systems.

Compensation and incentives include a salary range generally between $125,000 and $175,000, determined by factors such as local labor markets, education, and work experience, alongside opportunities for performance-based increases and productivity incentives. Eligible individuals who have not previously participated in this program may also qualify for a $40,000 student loan repayment or a $30,000 sign-on bonus.
