Optum Home & Community Care is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Optum Care at Home team in Waukesha, Jefferson, and Dane Counties, WI. This role offers an exciting opportunity to deliver preventive, integrated care directly to patients in their place of residence, helping them navigate the health care system and stay well at home.

This position requires daily travel to patient homes within the assigned Wisconsin counties. Qualified candidates may also be eligible for a $40,000 student loan repayment or a $20,000 sign-on bonus, subject to program terms and previous participation history.

Key responsibilities include:

  • Conducting comprehensive, in-home assessments for high-risk health plan beneficiaries.
  • Managing members through acute medical situations, chronic condition exacerbations, or short-term sub-acute rehabilitation episodes, including diagnostics and interventions.
  • Communicating and collaborating closely with an interdisciplinary care team.
  • Ensuring accurate and complete documentation, including ICD-10 conditions.
  • Facilitating advanced illness and advanced care planning conversations while providing patient and caregiver counseling and education.

Required qualifications for this position:

  • Graduate from an accredited NP or PA program with active certification through the AANP, ANCC, or NCCPA.
  • Active and unrestricted State Nurse Practitioner (NP) license or Physician Assistant (PA-C) in Wisconsin.
  • Current active DEA licensure/prescriptive authority or the ability to obtain it post-hire per state regulations.
  • Ability to complete physical requirements, such as moving a 30-pound bag in and out of a vehicle and navigating stairs and varied home environments.
  • Willingness to travel within the assigned area with a valid driver’s license and access to reliable transportation.

Preferred qualifications:

  • 1+ years of clinical experience in practice, preferably in a community or long-term care setting.
  • Experience meeting the medical needs of patients with complex behavioral, social, and/or functional needs, or working in non-clinical settings.
  • Proficient computer skills, including electronic medical record documentation.

Compensation for this role generally ranges from $109,500 to $164,000, determined by factors such as local labor markets, education, and work experience, alongside comprehensive benefits, retirement contributions, and incentive programs.
