Nurse Practitioner or Physician Assistant (Per Diem) – Hartford & Middlesex Counties – Senior Community Care

Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner or Physician Assistant for a per diem role serving Hartford and Middlesex Counties. Join our Senior Community Care team to provide holistic, patient-centered care directly to individuals in their homes, nursing homes, assisted living facilities, or senior housing settings. This life-changing work adds an essential layer of support to improve healthcare access and outcomes for Medicare and Medicaid patients.

In this per diem advanced practice role, you will deliver periodic and intermittent medical care, manage chronic and acute conditions, and conduct comprehensive patient assessments. Responsibilities include:

• Delivering cost-effective, high-quality primary care in collaboration with physicians and specialty providers.
• Performing thorough health assessments, ensuring accurate ICD-10 coding, and maintaining compliant documentation according to CMS regulations.
• Conducting initial, annual, and post-hospitalization medication reviews and reconciliations.
• Facilitating care coordination and treatment plan implementation with facility staff, families, and primary care providers.
• Promoting the Senior Community Care program within assigned facilities and communicating effectively with key stakeholders and internal teams.
• Participating in quality improvement initiatives, mandatory educational training, and maintaining required work availability.

To qualify for this position, candidates must meet the following requirements:

• Active national NP certification (ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care) OR PA certification through the NCCPA.
• An active and unrestricted Advanced Practice Clinician license in the State of Connecticut.
• Current active DEA licensure or the ability to obtain it post-hire per state regulations.
• A valid driver’s license, reliable transportation for travel between care sites within the designated area, and the physical ability to carry equipment and navigate various dwelling configurations.
• Availability to work at least 24 hours per month, with a portion of those hours during off-hours such as evenings, weekends, or holidays, up to a maximum of 960 hours per calendar year.

Preferred qualifications include prior hands-on post-graduate experience within long-term care, a strong understanding of geriatrics and chronic illness, familiarity with advanced illness and end-of-life discussions, and proficiency in electronic medical record documentation.

External candidates may be eligible for a sign-on bonus. Compensation for this per diem role ranges from $44.09 to $78.70 per hour, determined by factors such as education, local labor markets, experience, and certifications.
