Optum is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Senior Community Care (SCC) program as a Dual Statewide Traveler based in Providence, RI. This position requires regular travel throughout the states of Rhode Island and Connecticut to provide holistic, integrated care to our highest-risk health plan members in facility settings such as nursing homes, assisted living facilities, and senior housing.

As part of an interdisciplinary team, you will help bridge physical, mental, and social needs to create a seamless health journey for patients. Enjoy a balanced schedule with this role, which features no on-call shifts, no weekend requirements, and no holiday commitments.

Key responsibilities include:

Providing clinical coverage for primary Advanced Practice Clinicians (APCs) during periods of PTO or absences.
Managing members through acute medical situations or chronic condition exacerbations, including diagnostics and interventions.
Conducting comprehensive, routine, and acute care visit assessments while ensuring accurate and complete documentation of ICD-10 conditions.
Managing members during short-term, sub-acute rehabilitation episodes.
Communicating and collaborating closely with the interdisciplinary care team.

Required qualifications and credentials include:

For Nurse Practitioners: Graduate of an accredited MSN program, board certified through the AANP or ANCC (Adult-Gerontology, Adult, Family, or Gerontology), with preferred certification as an ANP, FNP, or GNP.
For Physician Assistants: Graduate of an accredited PA degree program and current board certification by the NCCPA.
Active and unrestricted APC license in either Connecticut or Rhode Island, with the ability to obtain the license for the alternate state.
Current, active DEA licensure or the ability to obtain it post-hire, along with the ability to gain a collaborative practice agreement if applicable.
At least 1 year of clinical experience in practice, with a background in long-term care preferred.
The ability to complete physical requirements, including moving a 30-pound bag and performing physical assessments, alongside access to reliable transportation.

Preferred qualifications include experience meeting the medical needs of patients with complex behavioral, social, and functional needs, a strong understanding of geriatrics and chronic illness, and proficiency in electronic medical record documentation.

Incentives and compensation for this full-time role include:

A $30,000 Sign-on Bonus or $40,000 Student Loan Repayment Bonus for eligible individuals who have not previously participated in this program.
An annual salary range from $89,900 to $160,600, determined by factors such as local labor markets, education, work experience, and certifications.
