Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated Nurse Practitioner or Physician Assistant for a Senior Care Per Diem position in Providence, RI. As an Advanced Practice Clinician in our Senior Community Care product, you will provide intermittent or periodic medical care services to patients in their homes, nursing homes, or assisted living and senior housing environments. This rewarding role adds a vital layer of support to improve healthcare access for Medicare and Medicaid patients.
Key responsibilities include:
- Delivering cost-effective, quality primary care to assigned members, managing both chronic and acute conditions in collaboration with physicians and specialty providers.
- Performing comprehensive assessments, conducting medication reviews and reconciliations, and ensuring accurate ICD-10 coding and documentation compliant with CMS regulations.
- Facilitating and evaluating the implementation of members’ care plans by engaging facility staff, families, and healthcare teams to prevent unnecessary hospitalizations.
- Coordinating care as members transition through different settings and managing ancillary services according to medical necessity.
- Communicating regularly with internal and external stakeholders, promoting the program in assigned facilities, and maintaining compliance with mandatory training and scheduling requirements.
To qualify for this per diem opportunity, candidates must meet the following requirements:
- Active national NP certification (ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care) or PA certification through the NCCPA.
- Active and unrestricted APC license in the State of Rhode Island.
- Current active DEA licensure and prescriptive authority, or the ability to obtain post-hire.
- A valid driver’s license and access to reliable transportation for travel to patient sites within the designated area.
- Availability to work a minimum of 24 hours per month.
- One or more years of hands-on post-graduate clinical experience.
Preferred qualifications include an understanding of geriatrics, chronic illness, and advanced illness discussions, along with proficient computer skills and adaptability to change.
The hourly pay for this role ranges from $43.22 to $77.21 per hour, determined by factors such as education, local labor markets, and work experience. Eligible team members may also participate in incentive programs and retirement contributions based on program guidelines.
