As part of our Senior Community Care (SCC) team, you will provide vital care to patients at home, in nursing homes, and in assisted living environments across Cumberland and York Counties, ME. In this longitudinal, integrated care delivery role, you will be part of an interdisciplinary team providing care to our highest-risk health plan members. Primary responsibilities include:

• Conduct comprehensive assessments for senior patients in home and facility settings.
• Manage members through acute medical situations or chronic condition exacerbations, including ordering and interpreting testing, diagnostics, and interventions.
• Manage members during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10 conditions and electronic medical records.
• Communicate and collaborate effectively with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with ongoing counseling, education, and support.

Required qualifications:

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Active and unrestricted license in your state of residence, or the ability to obtain by the start date.
• Active Nurse Practitioner certification through a national board (AANP or ANCC) in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nurse Practitioner.
• Current, active DEA licensure and prescriptive authority or the ability to obtain post-hire per state regulations.
• Ability to gain a collaborative practice agreement, if applicable in your state.
• Ability to complete physical requirements of the job, including moving a 30-pound bag in and out of a vehicle, navigating stairs and diverse dwelling conditions, and performing physical assessments.
• Valid driver’s license and access to reliable transportation.

Compensation for this specialty generally ranges from $109,500 to $164,000, including base pay and potential incentives based on factors such as local labor markets, education, and experience.
