We are currently seeking a dedicated Physical Therapist to join Geisinger Home Health, part of LHC Group and the Optum family of businesses. Now offering an exciting $10,000 sign-on bonus, this role invites you to be part of a leading post-acute care partner dedicated to helping patients feel their best while fostering a culture of caring, connecting, and growing together.
In this home health position, you will perform comprehensive assessments and evaluations of patient care needs, focusing on functional status, mobility, endurance, stamina, and pain management. Working collaboratively with physicians and the broader healthcare team, you will help determine individualized treatment plans, deliver targeted interventions, and continuously evaluate patient progress.
Primary responsibilities for this role include:
- Evaluating patient functional status and physical therapy needs to assist in developing the plan of care.
- Providing professional services within the scope of state practice laws and in coordination with the care team.
- Observing and reporting patient responses to treatment and any condition changes to the supervising nurse or physician.
- Treating patients to relieve pain, develop and restore function, and maintain maximum functioning.
- Instructing and informing patients, families, caregivers, and team members regarding the care plan, limitations, and goal progress.
- Visiting patients according to the Plan of Treatment and completing timely progress notes and evaluations per policy.
Required qualifications for applicants:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
- Ability to function professionally in any home environment regardless of the client’s age, race, background, or financial condition.
The hourly pay for this role ranges from $34.23 to $61.15 per hour, with actual compensation determined by factors such as local labor markets, education, work experience, and certifications. Eligible employees may also participate in a comprehensive benefits package, incentive programs, equity stock purchase, and 401(k) contributions, subject to eligibility requirements.
