As a Home Health Physical Therapist, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on your evaluation, you will help determine a treatment plan, perform interventions aimed at improving and enhancing the patient’s well-being, and evaluate ongoing progress.

Primary responsibilities include:

• Evaluating the patient’s functional status and physical therapy needs, and assisting the physician in developing the therapy plan of care.
• Providing services within the scope of practice defined by state laws and in coordination with other members of the health care team.
• Observing and reporting the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
• Instructing and informing the health care team, patient, and family or caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visiting patients according to the plan of treatment, completing progress notes for each visit, and submitting them per company policy.
• Completing all patient evaluations and developing the physical therapy plan of care within state-specific guidelines.

Required qualifications for this role include:

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

The hourly pay for this role ranges from $34.23 to $61.15 per hour based on full-time employment. Pay is determined by several factors including local labor markets, education, work experience, and certifications.
