As a Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status and mobility, endurance and stamina, pain, and other medical conditions. Based on your evaluations, you will help determine treatment plans, perform interventions aimed at improving and enhancing patient well-being, and evaluate ongoing progress.

Primary Responsibilities:

• Evaluate the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
• Provide services within the scope of practice defined by state laws governing physical therapy, in accordance with the care plan and in coordination with the health care team.
• Observe and report the patient’s response to treatment and any condition changes to the supervising nurse and/or physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
• Instruct and inform participating health care team members, patients, and family or caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visit patients according to the Plan of Treatment, complete a progress note for each visit, and submit notes per company policy.
• Complete all patient evaluations and develop the physical therapy plan of care within state-specific guidelines, reporting outcomes, goals, and anticipated visit frequency.

Required Qualifications:

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle, or reliable public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Compensation and Benefits:

• Annual total cash target pay ranges from $77,400 to $171,000 for full-time employment (40 weekly hours) at full productivity.
• Per visit point pay ranges from $44.65 to $98.65, and the hourly rate ranges from $37.21 to $82.21, depending on the specific activity performed (such as direct patient visits versus orientation).
• Total cash compensation is based on factors including local labor markets, education, and work experience, and may increase over time based on productivity and performance.
