As a Home Health Physical Therapist, you will play a vital role in delivering quality care directly to patients in their homes. Your primary responsibilities will include conducting thorough assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on your evaluations, you will collaborate with physicians and the healthcare team to develop, implement, and update comprehensive treatment plans aimed at improving and enhancing patient well-being.
Key responsibilities for this role include:
- Evaluating patient functional status and physical therapy needs to assist physicians in developing individualized plans of care.
- Providing professional physical therapy services within the scope of practice and in coordination with other multidisciplinary health care team members.
- Observing and reporting patient responses to treatment and any condition changes to the supervising nurse and physician.
- Treating patients using acceptable standards of practice to relieve pain, develop and restore function, and maintain maximum functioning.
- Instructing and informing patients, families, caregivers, and team members regarding the plan of care, functional limitations, and progress toward goals.
- Completing timely progress notes for each visit and submitting them according to established agency policies.
To qualify for this rewarding position, candidates must meet the following requirements:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, reliable vehicle insurance, and access to a dependable vehicle or reliable transportation.
- The ability to function effectively in any home environment, regardless of the client’s age, race, creed, color, sex, disability, or financial condition.

The hourly pay for this full-time role ranges from $34.23 to $61.15 per hour, with actual compensation determined by factors such as local labor markets, education, work experience, and certifications.
