As a Physical Therapist in Home Health, you will play a vital role in delivering compassionate, patient-centered care directly within the community, creating meaningful connections with patients, their families, and fellow team members.
In this role, you will be responsible for completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. You will work closely with physicians and the healthcare team to develop and implement effective treatment plans aimed at improving and enhancing patient well-being while continuously evaluating patient progress.
Primary Responsibilities:
- Evaluates the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
- Provides services within the scope of practice defined by state laws and in accordance with the established plan of care.
- Observes and reports the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instructs and informs the health care team, patients, and family caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visits patients according to the Plan of Treatment, completing and submitting progress notes per LHC policy.
- Completes all patient evaluations, establishes the physical therapy plan of care within state-specific guidelines, and reports outcomes, goals, and anticipated frequency of care.
Required Qualifications:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
- Ability to function professionally in any home environment regardless of client age, race, creed, color, sex, disability, or financial condition.
Compensation for this role ranges from $34.23 to $61.15 per hour, with actual pay determined by factors such as local labor markets, education, work experience, and certifications.
