As a Physical Therapist (PT), you will play a vital role in providing patient-centered care and evaluation within a home health setting. While our broader clinical teams address various aspects of patient rehabilitation, your focus will be on delivering professional physical therapy services that align with established plans of care and support optimal patient recovery and mobility.
Primary responsibilities generally include:
- Evaluating patient levels of function using appropriate assessments and diagnostic tests
- Assisting physicians and the healthcare team in the development and implementation of individualized therapy plans of care
- Providing physical therapy interventions aimed at improving and enhancing the patient’s functional mobility and overall well-being
- Observing, recording, and reporting patient responses to treatment and any changes in condition to the supervising nurse and/or physician
- Instructing patients, family members, caregivers, and other members of the healthcare team in supportive physical therapy techniques
- Participating in a weekend rotation as part of ongoing patient care delivery
Required qualifications for this role:
- Appropriate state licensure in therapy in the state of residence
- Current CPR certification
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
- Availability to work a weekend rotation
Compensation for this role ranges from $34.23 to $61.15 per hour based on full-time employment, with exact pay determined by factors including local labor markets, education, work experience, and certifications.
