In this home health role, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. You will help determine treatment plans, perform interventions aimed at improving patient well-being, and evaluate ongoing progress.

Primary Responsibilities Include:

  • Evaluating the patient’s functional status and physical therapy needs, and assisting the physician in developing the therapy plan of care.
  • Providing services within the state scope of practice, coordinating with the broader health care team.
  • Observing and reporting the patient’s response to treatment and condition changes to the supervising nurse and/or physician.
  • Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
  • Instructing patients, families, caregivers, and team members regarding the plan of care, functional limitations, and progress toward goals.
  • Visiting patients according to the plan of treatment, completing progress notes for each visit, and submitting documentation per company policy.
  • Completing patient evaluations and developing the physical therapy plan of care within state-specific guidelines, reporting outcomes, goals, and anticipated care frequency.

Required Qualifications:

  • Current Physical Therapy licensure in the state of New Mexico.
  • Current CPR certification.
  • Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Compensation and Benefits:

  • The hourly pay for this full-time role ranges from $34.23 to $61.15 per hour, based on factors including local labor markets, education, work experience, and certifications.
  • Comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, subject to eligibility requirements.
