In this Home Health role, you will be responsible for completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. You will help determine personalized treatment plans, perform interventions aimed at enhancing well-being, and evaluate ongoing patient progress.
Primary Responsibilities:
- Evaluate patient functional status and physical therapy needs while assisting the physician in developing the therapy plan of care.
- Provide services within your scope of practice in coordination with other healthcare team members.
- Observe and report patient responses to treatment and any condition changes to the supervising nurse or physician.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning.
- Instruct patients, families, caregivers, and team members regarding the plan of care, functional limitations, and progress.
- Visit patients according to the Plan of Treatment, complete progress notes for each visit, and submit documentation promptly.
Required Qualifications:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and dependable transportation or access to public transportation.
- Ability to function professionally in any home environment.
Preferred Qualifications:
- Ability to manage multiple tasks simultaneously while working independently.
- Strong communication, writing, and organizational skills.
The hourly pay for this full-time role ranges from $34.23 to $61.15 per hour, with actual compensation determined by factors such as education, experience, local labor markets, and certifications.
